Home Health RN – Nurse Resident Oversight
Location: Barre, VT
Start Date: November 16, 2026
Contract: 26 Weeks
Schedule: Monday–Friday, Day Shift

Job Highlights:

Provide individualized, goal-oriented nursing care to patients in their homes.
Manage patient caseloads and coordinate care as the lead discipline when applicable.
Support and mentor a Nurse Resident in developing clinical skills and confidence.
Work with Central Vermont Home Health & Hospice, a community-focused, not-for-profit healthcare organization.
Mileage reimbursement at the IRS rate for travel between patients (commute excluded).

Requirements:

Minimum 1 year of nursing experience.
Active state RN license.
BLS/BCLS certification.
Home Health, Hospice, or Case Management experience preferred.
EMR: Homecare Homebase (HCHB).
