Facility: San Angelo State Supported Living Center
Location: 11640 US Highway 87 N, Carlsbad, TX 76934

Openings: 3
Contract Duration: Minimum 6 months
Approximate Start Date: November 16, 2026
Schedule: Rotating schedule, 4:00 PM–12:00 AM, 5 days per week
Hours: 40 hours per week; no overtime
Weekend Schedule: Every third weekend off

Position Summary:

San Angelo State Supported Living Center is seeking three qualified Licensed Vocational Nurses (LVNs) or Licensed Practical Nurses (LPNs) to provide nursing care and support to individuals receiving services at the facility. The nurse will administer medications and treatments, perform nursing procedures, monitor individuals' health conditions, maintain accurate medical records, and collaborate with other healthcare team members to ensure patient safety and quality care.

Key Responsibilities:

• Administer medications and treatments orally, rectally, internally, or by injection, as prescribed.
• Perform technical nursing procedures in accordance with facility and state regulations after competency has been demonstrated.
• Observe, document, and report relevant changes in individuals' health conditions.
• Communicate important clinical information to other healthcare team members verbally and in writing.
• Respond to emergency situations and take appropriate action to maintain individual safety.
• Follow facility safety and infection-control procedures.
• Monitor individuals during treatments and procedures.
• Collect specimens for laboratory testing and perform appropriate screening tests when requested by laboratory personnel and after competency has been documented.
• Educate individuals regarding specific health goals.
• Assist other staff with turning and repositioning individuals to administer treatments.
• Ensure nursing supplies are available and appropriately maintained.
• Maintain accurate medical records using the Electronic Health Record (EHR) or paper charts.
• Attend work regularly and comply with assigned schedules, including rotating shifts, extended shifts, or overtime if required by the agency.

Minimum Qualifications:

• Active Texas LVN license or an LPN license from a state that participates in the Nurse Licensure Compact and recognizes the applicable multistate privilege.
• Ability to communicate effectively, both verbally and in writing.
• Computer and Electronic Health Record (EHR) skills preferred.
• Experience working with individuals with intellectual and developmental disabilities (IDD) and behavioral or psychiatric conditions preferred.
• Valid photo identification.
• Current American Heart Association (AHA) CPR certification.

Compliance Requirements

• Negative PPD results within the past 12 months.
• Negative COVID-19 test; testing will be provided by the facility.
• Successful urine drug screening and background check, arranged by the candidate.
• Successful FBI fingerprint-based background check, provided by the facility.
• All required compliance must be completed before the start date.
