2. License and Certification Requirements

• Active Occupational Therapist license in Washington State.
• Graduation from an accredited Occupational Therapy program.
• Current BLS certification issued by the American Heart Association (AHA).
• Strong communication skills and ability to work independently and with a team.

3. Job Responsibilities

• Evaluate patients’ functional abilities.
• Develop individualized treatment plans.
• Provide hands-on therapy according to physician orders and facility protocols.
• Document patient progress and maintain accurate records.
• Collaborate with PTs, speech-language pathologists, nurses, and other healthcare professionals.
• Educate patients and families on safety and continued care.
• Follow state and federal regulations.

5. Required Submission Documents

• Resume: Current full name, employment history for the last 7 years, facility name, city/state, title, department, employment dates, and education details.
• Skills checklist: Completed within the past year for the relevant specialty.
• Two professional references: From the last 3 years, including at least one supervisor. Must include full name, facility, matching employment dates, skills/strengths, and ratings. Letters of recommendation are not accepted.
• License verification: Completed within 30 days of submission; Nursys accepted where applicable.
• Current AHA BLS certification.
• Background checks: OIG and SAM.
• Pre-employment physical, TB testing and questionnaire, immunity records, drug screen, and competency exam.
• Joint Commission Core I, II, and III.
• Valid state-issued driver's license or ID.
• Washington DSHS Background Check Authorization Form.
• Employment application and pre-hire authorization.
• COVID documentation and flu vaccine documentation or the required election form.
• Healthcare provider timekeeping instructions.
