Local Radius rule:  No locals -- All candidates must be at least 50 miles from the facility (based on driver's license)

Employee Separation Policy:  Any past employee of UPH whether FTE, or PRN for the UPH float pool, cannot work travel in another Market no matter which facility they currently work at, or have worked at within the past 12 months.  If they worked travel at UPH (not FTE or PRN), the separation policy does not apply

HOLIDAY update: Unless the Unit is closed, your contractor is required to work 1-2 of 3 upcoming Winter holidays

Orientation schedule: 11/2, 11/16

SUBMISSION REQUIREMENTS
 

• 36 or 48 hour contracts available
• 8, 10, 13 week assignments available
• At least 1 travel contract completed [preferred]
• Epic experience [required]
• State license [required]
• Cardiac experience [required]
• ACLS [preferred]
• BLS [required]
• NIHSS [required]
• New grads accepted
• Not an employee of UnityPoint in the last 12 months [required]
• Drips on unit: Med1: Heparin, Cardizem, Nitro, Non-DKA insulin, Amio

• Work history/resume [required]
• Skills checklist (must have been completed within 1 year of submission date) [required]
• References - 1 supervisory reference required at time of submission [required]
• License verification (must be less than 30 days old) [required]
• Copy of required certifications [preferred]
• Driver’s License **Must have taxable address outside of 50 miles from facility** [required]
