Trauma Program Social Worker III
Position Overview
We are seeking a Trauma Program Social Worker III to provide psychosocial assessment, crisis intervention, patient and family support, and discharge planning for patients with traumatic injuries.
You will work with a multidisciplinary trauma team, supporting patients from admission through discharge across the Emergency Department, inpatient units, and Intensive Care Units, with continued follow-up after hospitalization.
• Trauma certification
• Experience supporting trauma patient populations
• Trauma-informed care experience
• PTSD screening and behavioral health follow-up experience
• Crisis intervention and family support experience
• Schedule can be 8-4 or 9-5. This is direct hire role
