Position: LCSW

Location: Glendale, CA

Setting: Inpatient Hospital

Duration: 13-weeks

Compensation

Travel Package: $2,862/week

Position Details

Schedule:

5x8s, day shift

Job Responsibilities

Discharge planning and care coordination
Psychosocial assessments
Hospice and post-acute care planning
Patient/family counseling and support
Community resource referrals
Collaboration with physicians, nursing, and interdisciplinary teams

Requirements

Active CA LCSW license is needed of all applicants!
2 yrs experience working as an LCSW in an acute care hospital setting
EPIC EMR experience is preferred
