Job Title: Master's Social Worker (MSW) – Hospice Facility Type/Setting: Hospice Shift &amp; Schedule: Day Shift | 8:00 AM–4:30 PM | 5×8-hour shifts Weekly Hours: 40 hours Contract Length: 13 weeks Requirements:<ul><li>Master's degree in Social Work (MSW) required.</li><li>Minimum 2 years of clinical, emergency department (ED), or medical social work experience.</li><li>Must be working toward Licensed Clinical Social Worker (LCSW) licensure, at minimum.</li><li>Active driver's license required.</li><li>Active BLS certification required.</li><li>Epic EMR experience.</li><li>Must be able to lift 50 lbs intermittently.</li><li>Must have a vehicle suitable for driving in various conditions, including gravel roads, snow, rain, and ice.</li><li>Must be able to work with patients of all ages, with the majority over 65 years old.</li></ul>Responsibilities:<ul><li>Conduct biopsychosocial assessments for hospice patients.</li><li>Provide social work services to patients and families across all age groups.</li><li>Support patients and families with psychosocial needs associated with hospice care.</li><li>Document patient assessments and care in the Epic EMR.</li><li>Travel to patient locations under varying road and weather conditions.</li></ul>
