Job Title: Licensed Master Social Worker (LMSW)
Facility: NYC Health + Hospitals – McKinney
Location: Brooklyn, NY 11203
Job Type: Contract – 14 Weeks
Start Date: November 17, 2026
End Date: February 20, 2027
Schedule: Monday–Friday, 9 AM–5 PM
Hours: 35 hours/week
Flexibility: 8 AM–4 PM or 8:30 AM–4:30 PM
Weekend: One Saturday per month
Job Description:
Seeking an experienced LMSW to work in a 40-bed short-term sub-acute unit. Responsibilities include completing patient assessments, coordinating discharges, assisting with advance directives, and providing social work support to patients and families.
Requirements:
- Master's Degree in Social Work (MSW).
- Active NYSED LMSW license with Primary Source Verification (PSV).
- Minimum 1 year of LMSW experience.
- 6 months–1 year of nursing home experience as a social worker required.
- Experience with patient assessments, discharge planning, and advance directives.
- Strong communication and documentation skills.
Preferred Experience: Nursing home, long-term care, or sub-acute social work experience.
