• 40 hours
• 5 x 8 ’s
• Shift: Days
  • 9am-5:30pm with alternating weekends
• Patient ratios (if known): Within Guidelines of CA State Law
• Float Requirements: None
• Is RTO Accepted? What is the Max: Up to 4 days max RTO approved
• On Call Requirement? NO
• Certs: BLS, LA County Fire Card
• License: Active CA LVN License
• If CA License is expired, it will need to be renewed
• Will accept local travelers

Experience:

• 1 year of Case Manager experience and 2 years of LVN experience
• Primary focus: Patient placement/discharge planning — the majority of the job is coordinating where patients go next and ensuring a smooth transition.
• Required: Strong working knowledge of Skilled Nursing Facilities (SNFs) and common post-acute placement options.
• Insurance awareness: Understanding how insurance impacts placement decisions (used by the Case Manager to secure/approve appropriate placements).
• Placement types to know:
  • Acute rehab
  • Board & care (boarding care)
  • SNF/SNF placement
• Eager to learn, coachable, and open to being trained/developed.
• Required: Strong level of familiarity with Epic
• Required: Proven expertise in complex discharge dispositions and care transitions.
• Required: Able to consistently use the appropriate terminology associated with discharge planning
  • Client will ask questions like this during interview and LVN must be knowledgeable
• More of a Coordinator - nothing bedside, someone who has experience with placements, dispositions, home comfort.
• Travel Experience not required but preferred
• Previous employees who were terminated will not be accepted for submission or placement 

Orientation: The LVNs will complete a structured 5-day training program, which includes 2 days of required online education followed by 3 days of orientation with a preceptor to support hands-on learning and integration into the team. 

COVID Vaccine Requirement: No longer require COVID-19 vaccines, and candidates will not need boosters. They will accept candidates who have never been COVID-19 vaccinated.
