Job Title: Home Care Registered Nurse (RN)
Location: Bennington, VT 05201
Assignment Length: 13 Weeks
Number of Openings: 2
Shift: Monday–Friday, 8:00 AM–4:00 PM

Additional Coverage: Some weekends, on-call, and holidays required
Minimum Experience: 2 Years
 

Key Responsibilities:

• Perform home health nursing assessments and develop individualized patient plans of care.
• Complete OASIS documentation accurately and within required timelines.
• Manage assigned patient caseloads and coordinate patient visits.
• Provide skilled nursing care, including IV therapy, wound VAC management, and complex wound care.
• Collaborate with physicians, LPNs, and interdisciplinary care teams.
• Educate patients and caregivers on medications, treatment plans, and discharge instructions.
• Coordinate patient admissions, transfers, and safe discharges.
• Maintain accurate clinical documentation using McKesson.
• Participate in case conferences, peer reviews, and quality improvement activities.

Must-Have Requirements:

• Minimum 2 years of nursing experience.
• Home Health nursing experience with strong OASIS documentation knowledge.
• Experience with patient assessments, care planning, and case management.
• Skilled in IV therapy, wound care, and wound VAC management.
• Ability to work independently and coordinate patient care.
• Willingness to provide hospice coverage when on-call.
• Availability for some weekends, on-call rotations, and holiday coverage as required.
