Description
Seeking an experienced LVN for an extendable 13-week Case Management assignment focused on patient placement, discharge planning, and coordinating post-acute care transitions. This is a primarily administrative/coordinator role with no bedside responsibilities.
- Contract: 13 weeks
- Schedule: 5x8-hour shifts, 9:00 AM – 5:30 PM
- Hours: 40 hours per week
- Weekends: Alternating weekends required
- Patient Ratios: Per California state guidelines
- Floating: None
- RTO: Up to 4 days
- On-Call: None
- Local Travelers: Accepted
Requirements:
- Active California LVN license
- BLS certification and LA County Fire Card
- Minimum 2 years of LVN experience
- Minimum 1 year of Case Management experience
- Strong working knowledge of Epic required
- Proven experience with complex discharge planning and patient dispositions
- Familiarity with SNF, acute rehab, and board-and-care placements
- Understanding of how insurance impacts patient placement and discharge decisions
- Strong knowledge of discharge planning terminology and care transitions
Ideal Candidate: Someone with strong coordination and placement experience who is comfortable navigating post-acute care options, insurance requirements, and complex discharge dispositions. Travel experience is preferred but not required.
Note: This is a non-bedside role focused primarily on discharge coordination and patient placement.
