Description

Float between three ministries: PWF, PMH, and PNB.

Emergency Department behavioral health assessments.

Behavioral health assessments for all age groups.

Care management and discharge planning on medical/surgical floors.

Coordinate discharge planning and interdisciplinary care.

Active Oregon LCSW license or compact license in hand at submission, as applicable. Minimum 1 year of experience. Epic charting experience. Case management. Discharge planning. Emergency Department experience preferred. Acute behavioral health assessment experience preferred. Community hospital experience preferred. Trauma Level I/II experience preferred.
