1. Required Qualifications

• Minimum 1 year of experience.
• ARRT Mammography (M).
• ARRT Radiography (R).
• BLS.
• Active state license required.
• Compact license must be in hand at submission.
• First-time travelers accepted with relevant experience.
• Outpatient mammography experience preferred/required by the listed skill profile.

4. Scheduling & Time-Off Rules

• RTO maximum: 7 days.
• All RTO and scheduling requests must be disclosed at submission.
• Post-submission or post-offer RTO may not be approved.
• Candidate must commit to working 3 of the 5 holidays:
  • Thanksgiving
  • Christmas Eve
  • Christmas Day
  • New Year's Eve
  • New Year's Day

5. Important Submission Requirements

- Candidate must disclose whether they have previously worked at any Providence facility as:

• Permanent employee
• Travel clinician
• Registry/per-diem

If yes, provide facility location and dates worked.

• 2 references covering at least 1 year within the last 3 years.
• Candidate's background must meet the facility's policy; facility does not accept Nursys or background-check hits.
• Only one Providence facility submission at a time.
• Local radius: 50 miles.
• Local candidates accepted at a lower bill rate.
• Firm bill rate.
• Return staff may return after 3 months.

6. Orientation & Compliance

• Modules are non-billable.
• Average module time: 16 hours.
• More than 20 hours requires PM approval.
• Modules completed before start and annually.
• Time is self-reported and recorded in Workramp transcripts.
