Description

Home Health RN | Monday–Friday

Schedule: 8:00 a.m.–4:00 p.m.
Provide skilled nursing care to patients of all ages in their homes.
Assess physical, psychosocial, environmental, and pain-management needs, including stable and emergent situations.
Develop, implement, and evaluate individualized plans of care.
Coordinate services with patients, families, physicians, referring agencies, and the interdisciplinary team.
Educate patients and caregivers on medications, diet, home safety, infection prevention, and self-care.
Maintain timely, accurate documentation and monitor payer requirements.
Supervise and provide clinical guidance to home health aides, STNAs, and LVNs.
Arrange referrals and participate in interdisciplinary care meetings.
