Description
POSITION TITLE
- Clinical Pharmacist – Ambulatory Care
REQUIRED EXPERIENCE
- 4 years of direct patient care experience demonstrating medication management required.
- PGY2 Ambulatory Care residency required.
- Experience providing clinical pharmacy services in primary care / ambulatory care settings is essential.
- Must be able to manage approximately 16 patients per day.
- Precepting pharmacy students or residents is part of the role.
SCHEDULE
- Shift options:
- 7:00 AM–5:00 PM
9:00 AM–7:00 PM
REQUIRED CERTIFICATIONS
- BCACP – Board Certified Ambulatory Care Pharmacist required.
- BLS required.
- PGY2 Ambulatory Care residency required.
LICENSE
- Active Texas pharmacist license through the Texas State Board of Pharmacy required.
