Specialty: Inpatient Medical Social Worker / Case Management
Facility: Providence – PWF, PMH, and PNB (3 ministries)

1. Shift & Schedule

• Day Shift: 4×10 hours (7:00 AM – 5:00 PM)
• Night Shift: 4×10 hours (4:00 PM – 2:00 AM)
• Schedule: Rotating shifts; facility will try to provide a consistent pattern.
• Weekends: Alternating weekends.
• On-Call: No.
• Block Scheduling: Not guaranteed.
• Floating: Required between all three ministries; nearby sister facilities may also be included.
• Contract: Open to 13-week assignment.

3. Mandatory Requirements

Must Have

• Minimum 1 year of experience.
• Valid LCSW license.
• Compact license must be in hand at submission.
• Epic charting experience.
• Acute-care inpatient hospital experience.
• Medical/Surgical discharge planning experience.
• Emergency Department behavioral health assessment experience.
• Ability to float between three ministries.

Most important: Candidates must have BOTH:

1. Acute-care inpatient case management and discharge planning experience.
2. Behavioral health assessments in the Emergency Department.

4. Preferred Experience

• First-time travelers accepted.
• Community hospital experience.
• Level I or Level II trauma experience.
• Care management experience.
• Psychiatric / behavioral health experience.
• Experience assessing patients of all ages in the ED.
• Experience working across multiple facilities.

5. Clinical Responsibilities

• Conduct behavioral health assessments in the Emergency Department.
• Perform psychiatric assessments for adolescents, adults, and geriatric patients.
• Coordinate inpatient medical/surgical discharge planning.
• Manage patient transitions and care coordination.
• Collaborate with physicians, nurses, and interdisciplinary teams.
• Assist with placement and discharge arrangements.
• Provide social work support for acute-care patients.
