Description

Job Title: Clinical Pharmacist

Locations: Tuba City, AZ, 86045

Duration: 13 + Weeks Extension

Shift : 8:00 AM – 5:00 PM , Monday- Friday

Summary

We are looking for an experienced Clinical Pharmacist to provide pharmacy services and medication management to patients.
The pharmacist will ensure safe and accurate medication dispensing and provide clinical support to patients and healthcare staff.

Responsibilities

Review and verify medication orders.
Prepare and dispense medications safely.
Provide medication management for patients of all age groups.
Monitor medication safety and prevent medication errors.
Provide medication guidance to physicians and nursing staff.
Maintain accurate pharmacy records and documentation.
Follow hospital pharmacy policies and procedures.
Support daily pharmacy operations and patient care services.

Required Qualifications

Minimum 3 years of Clinical Pharmacist experience.
Doctor of Pharmacy (PharmD) or Bachelor of Pharmacy (BPharm) degree.
Current, valid, and unrestricted US State Pharmacy License.
Current Basic Life Support (BLS) certification.

Schedule Requirements Must be available for day and evening shifts. Weekend and holiday availability required. Rotational shifts.
