Position: Home Health OT
Shift: 5x8 Days 0800-1630
Certification: BLS
NO CALL
Scheduling Type: BLOCK
Patient Age Group: Geriatrics
Avg Daily Census: 650
Patient Diagnoses: HF, COPD, wounds, DM, post-surgical, respiratory illness, sepsis, dementia, s/p CVA, s/p MI
Job Summary Responsible for the evaluation and treatment of patients within the scope of Occupational Therapy, determining plan of care and setting goals of treatment. May provide oversight to therapy assistants, therapy aides and/or students in following the plan-of-care and treatment of patients. Works collaboratively as a part of a patient care team toward the best outcomes for patients. Essential Functions Evaluates assigned patients and establishes a treatment plan and goals, using data from a variety of sources, such as patient records, interviews, observation and team members.
