Registered Nurse – Home Health (RN)

Job Description:

We are seeking an experienced Home Health RN to provide skilled nursing care to patients in their homes. The nurse will conduct patient assessments, develop and implement individualized care plans, coordinate with interdisciplinary teams, and maintain accurate clinical documentation.

Key Responsibilities:

• Provide skilled nursing care through home visits.
• Conduct patient assessments and monitor health conditions.
• Develop, implement, and evaluate individualized care plans.
• Coordinate patient care with physicians and interdisciplinary teams.
• Educate patients and families on disease management and treatment plans.
• Maintain accurate and timely clinical documentation.
• Ensure quality patient outcomes and continuity of care.

Requirements:

• Active Registered Nurse (RN) license.
• Previous Home Health nursing experience preferred.
• Strong clinical assessment and patient care skills.
• Excellent communication and documentation skills.
• Ability to travel throughout the assigned service area.

Schedule & Hours:

• Full-time position.
• Monday–Friday schedule.
• First week: Approximately 32 scheduled hours due to orientation and start schedule.
• Subsequent weeks: 36 hours per week guaranteed for the duration of the contract.
