Location: Lakewood, California Role Type: Travel/Contract, Registered Nurse Contract Length: 13 weeks Shift: Day, 3x12 hours (7:00 to 19:30), 12 hours per day, 36 hours per week Float Required: Yes Weekend Coverage: Yes Candidate Eligibility: First-time travelers and local candidates are accepted

Required Qualifications

• Valid RN license in California
• Med Surg / Telemetry experience of 1 year, obtained within the last year
• ACLS (Advanced Cardiac Life Support) issued by American Heart Association or RQI
• BLS issued by American Heart Association or RQI
• NIHSS certification
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• Experience with Epic

Competencies

• Medication/pharmacology exam (required for RNs)
• Unit-specific competency exam
• Basic dysrhythmia/EKG exam (required for Tele RNs), with 80% passing score
• Infection control/bloodborne pathogens exam (full exam required, 80% passing score; CORE exams are not accepted)

Health Requirements

• Physical exam within 1 year of start, updated annually
• N95 fit test within 1 year of start, renewed annually. A PAPR is acceptable only with proof of a failed N95 fit test.
• TB screening: negative PPD, TSpot, or QuantiFERON within 1 year of start. If TB-positive, a negative chest X-ray within 4 years and an annual TB questionnaire are required.
• Immunity documentation for rubeola, mumps, rubella, and varicella: positive titer or 2 vaccines after age 1. Negative or equivocal titers require 2 boosters, and the candidate may start with the second booster pending.
• Hepatitis B series or declination (titer not acceptable)
• Tdap vaccine, or a pertussis titer showing immunity if the vaccine cannot be obtained for a health reason
• Hepatitis C titer strongly preferred; declination accepted if necessary, with a reason provided
• Flu vaccine by 12/5 for the 2026-27 season, or a signed declination. Unvaccinated candidates must follow universal masking on facility grounds.
• 9-panel drug screen within 1 year of start (instant drug screen acceptable)

Background and References

• Criminal background check within 1 year of start, updated annually, with a 7-year search covering statewide, nationwide, sex offender, SSN trace, and federal records
• Background hits involving open cases or DUI within the last 5 years result in automatic cancellation.
• 2 professional references completed within 1 year of start, in the current specialty, from a leadership role (department manager, supervisor, charge nurse, nurse manager, or assistant nurse manager)

Orientation and Paperwork

• Orientation safety manual acknowledgement and signed HIPAA acknowledgement
• Facility TB symptom review, required before start
• Confidentiality agreement, to be completed within 72 hours of signing
• Signed unit job description, orientation packet acknowledgment, and skills self-assessment
• Traveler acknowledgement covering Joint Commission readiness, physician rounding clinical updates, and national patient safety goals
• Epidural pain policy review (nothing to submit)
• Standard of care: skin care packet and attestation
• Sexual violence and sexual harassment policy review
• California Abuse and Neglect Reporting Act form, demographics and emergency contact form, and Lakewood orientation packet (required for Lakewood locations)
• ICU traveler tip sheet, if working in an ICU
• Agency badge required
• Facility orientation modules completed on site during Day 2 of orientation, with first-day instructions sent before start
