Location: Wichita, Kansas Employment Type: LTA Contract Length: 13 weeks Shift: 07:00 - 19:30 Schedule: Every other weekend, no call

Immunizations: Documented proof of 2 MMR doses or positive IgG titer (no declinations accepted). Hepatitis B series or signed declination. Varicella proof, immunity evidence, or history verification (no declinations accepted). Tdap, valid for 10 years (no declinations accepted).
Influenza vaccine: Required for the flu season from October 1, 2026 through March 31, 2027. Exemptions and declinations are accepted, but the candidate must wear a mask at all times.
TB screening: 2-step PPD or IGRA. Annual TB questionnaire. A positive result requires a chest X-ray within the last year.
Health statement: Statement of good health or physical exam before the first assignment, updated annually.
Color vision: Completed before start, updated every 3 years.
Fit test: Required and updated annually. PAPR fit tests are not completed onsite.
Drug screen: 11-panel drug screen within 30 days before the first assignment, repeated annually.

Criminal background check, including Social Security trace, OIG, and sex offender searches, completed within 90 days of the first assignment and updated yearly
EPLS check within 90 days of start
Education verification within 90 days of start (diplomas not accepted)
Third-party employment verification for the past 7 years, updated yearly
Confirmation of assignment on agency letterhead, provided at the initial assignment and each extension
Driver's license or state ID, not expired, showing front and back, with an address matching the file

Pre-hire education modules totaling 6 hours, with the attestation uploaded by the agency
Nursing contract worker onboarding, completed onsite
Nova StatStrip modules, required annually for non-allied positions
Badge photo: forward-facing headshot, white or blue background, no filters

Unit-specific checklist, which the unit leader must upload within 1 week if completed
Positive PPD with negative chest X-ray and annual TB questionnaire, if applicable
Legal name change document, if applicable
