Assignment Overview

• Travel/Contract role in Los Alamitos, California
• 13-week assignment
• Night shift: 3x12 hours, 19:00–7:30
• Call: as needed
• Float: as needed

Required Qualifications

• Valid RN license in California
• ICU - Intensive Care Unit (1 year of experience)
• ICU - Intensive Care Unit experience within the last year
• NIHSS
• Associate of Science in Nursing / Associate Degree in Nursing
• Experience with Epic
• 2 references within 1 year of start in the current specialty, from a leadership role
• Government issued photo ID
• Criminal Background Check within 1 year of start; must be updated annually
• Physical exam within 1 year of start; must be updated annually
• N95 mask fit testing within 1 year of start; must be renewed annually
• Negative TB test or chest X-ray within 1 year of start
• Hepatitis B series or declination
• Flu vaccine required by 12/5, or signed declination
• Tetanus/Diphtheria/Tdap
• Rubeola (Measles)
• Mumps
• Rubella
• Varicella
• Drug screen within 1 year of start
• Signed HIPAA acknowledgement
• CEUs from the last 24 months for RNs
• Unit-specific competency exam
• Infection Control/Bloodborne Pathogens exam
• Medication/Calculation Exam
• Arrhythmia Recognition Assessment for MS/Tele, Tele, Step Down, ER, ICU, IR, and PACU nurses
• Signed unit-specific job description
• Signed confidentiality agreement within 72 hours of signing
• Signed traveler acknowledgement
• Signed Epidural Pain Policy Review
• Signed Standard of Care: Skin Care attestation
• Signed Orientation Info & Acknowledgement
• Signed Skills Self Assessment
• Signed Pyxis Agreement
• Signed ICU Traveler Tip Sheet
• Signed Sexual Violence and Sexual Harassment Policy

Notes
