Required Qualifications

• Valid RN license in California
• Med Surg experience of at least 2 years
• Med Surg experience within the last year
• ACLS (Advanced Cardiac Life Support) issued by American Heart Association or RQI
• BLS (Basic Life Support) issued by American Heart Association or RQI
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing
• Experience with Epic

Onboarding Documents

• Government-issued photo ID
• Signed HIPAA acknowledgment
• Signed Orientation Safety Manual acknowledgment
• Review of the Epidural Pain Policy before start
• Standard of Care: Skin Care packet, with signed attestation
• Unit-specific job description, signed before start
• Confidentiality Agreement, completed within 72 hours of signing
• Traveler Acknowledgement covering Joint Commission Readiness, Physician Rounding Clinical Update, and National Patient Safety Goals for Hospitals
• Sexual Violence and Sexual Harassment Policy review
• Orientation packet and acknowledgment, signed in DocuSign
• Skills self-assessment, signed in DocuSign
• Pyxis agreement, due 5 business days before orientation
• Wage Benefit Parity form, required for each new travel assignment and not for extensions
• Lakewood-specific documents: CA Abuse and Neglect Reporting Act, Contract Staff Demographics and Emergency Contact Information, and Lakewood Orientation Packet
• Medical exemption or disability exception form, if needed, with facility clearance first

Orientation and Scheduling

• First-day instructions are emailed from the program manager on the Friday before start.
• Orientation modules are completed on Day 2, onsite or at home after activating the facility network ID. They must be finished before the candidate starts on the floor.
