Location: Los Angeles, California Employment Type: Travel Contract Length: 12 weeks Hours: 40 per week Shift: Monday to Friday, 7:30a-4p
Role Overview
Coordinates management of care and ensures optimum use of resources, service delivery, and compliance with external review agencies. Integrates case management, utilization review and management, and discharge planning to promote continuity of care and cost effectiveness. Provides ongoing assessment, care planning, plan implementation, and evaluation of individual patient needs while respecting patient preferences. Serves as a patient advocate by coordinating resources, planning discharge, and addressing the holistic needs of the patient.
Required Qualifications
- Valid RN license in California, attached to resume
- Associate of Science in Nursing (ASN) / Associate Degree in Nursing
- Basic Life Support (BLS) certification issued by the American Heart Association, attached to the submission packet
- Minimum 2 years of Case Management experience, with experience within the last year
- Experience in a Long Term Acute Care (LTAC) setting
- Experience with MCG
- Experience with discharge planning
