Schedule

Shift: 3 x 12-hour day shifts, 7:00 AM to 7:30 PM (scheduled 7 AM to 7 PM)
Hours: 36 per week
On-call: Call follows each 12-hour shift from 7 PM to 9 PM, every other weekend. On Sundays, the nurse takes call from 7 AM to 9 PM.
Guaranteed hours: The client may cancel up to 36 hours per 13-week assignment without paying for the cancelled hours. Cancellations beyond that are billed to the client. Shifts cancelled in lieu of being placed on call count toward this limit.
Schedule requests: Block schedule requests are open for discussion with the manager.
Time off: Requests are handled through the manager.

Required Qualifications

Active Illinois RN license, or be in the application process with ETA to apply
Minimum 2 years of dialysis experience
Dialysis experience within the last year
BLS, AHA (RQI acceptable)
Associate of Science in Nursing (ASN) / Associate Degree in Nursing

Onboarding Requirements

Background: Background check within 30 days of start, covering all locations lived or worked in the past 7 years, with aliases included, and updated annually. Also required: EPLS/GSA/SAM and HHS/OIG checks within 30 days prior to start, and education verification.
Illinois searches: Department of Corrections inmate search, child murderer/violent offender registry search, and most wanted search (must show all 20 fugitives), all within 30 days of start.
Fingerprinting: Illinois Healthcare Worker Registry fingerprinting applies to clinical candidates without an Illinois state license. Prints must be taken in Illinois 3 business days before start.
Drug screen: 9-panel drug screen within 30 days of start. Dilute results are not accepted and require a repeat test.
Physical exam: Within 1 year of start, signed by an MD, NP, PA, DC, APN, FNP, or DO. Must state the candidate is in good physical and mental health and able to perform core functions without restrictions.
TB: 2-step PPD, TSPOT, or Qgold within 1 year of start. TB questionnaire required annually.
Immunizations: Measles (rubeola), mumps, rubella, hepatitis B, and varicella, each by vaccine series, positive titer, or signed declination.
Flu: Seasonal flu vaccination or documented exemption required by 11/30. Declinations are not accepted. Flu season ends March 1.
Fit test: Required, with PAPR users exempt from documentation. Cannot be completed on site.
Orientation: Orientation safety manual acknowledgement; code of conduct signed within 7 days of contract signing; computer-based modules due by 4 PM CST on the first day of orientation; badge photo due by the Monday before start; badge name confirmation.
Other: Driver's license or state ID copy required. Labs must be completed or scheduled within 7 days of contract signing.
