Location: Lakewood, California Start: ASAP Contract Length: 13 weeks

Physical exam: Required within 1 year of start, updated annually
N95 fit test: Required within 1 year of start, renewed annually. If the candidate fails the fit test, documentation of a failed N95 fit test is needed to accept a PAPR
TB screening: Negative PPD, TSpot, or QuantiFERON-Gold within 1 year of start (PPDs marked negative are acceptable regardless of size). For a positive TB result, a negative chest x-ray within 4 years and an annual TB questionnaire are required
Tdap: Required. If the candidate cannot get it for a health reason, a Pertussis titer showing immunity is required. If immunity is not shown and the candidate continues to decline Tdap, the assignment will be cancelled
Measles (rubeola), mumps, rubella, and varicella: Positive titer, or 2 vaccines after age 1. Negative or equivocal titers require 2 boosters, and the candidate may start with the 2nd booster pending
Hepatitis B: Vaccine series or declination required. Titer is not acceptable
Hepatitis C: Titer is strongly preferred. A declination is accepted if necessary, and must include a reason and not be blank or "N/A"
Flu vaccine (2026-27 season): Required by 12/5. After 12/4, anyone without a current flu vaccine or signed declination will be pulled. Declinations are accepted, and onsite vaccines are available. Candidates who decline must follow seasonal masking guidelines

Criminal background check: Within 1 year of start, updated annually. Covers a 7-year search, including statewide, nationwide, sex offender, SS trace, and federal searches
Automatic cancellation: Open cases or DUI hits within the last 5 years
Photo ID: Government-issued photo ID required
Drug screen: 9-panel drug screen within 1 year of start. An instant drug screen is acceptable. Not required to be updated annually

Sign the HIPAA acknowledgement
Complete the confidentiality agreement within 72 hours of signing
Complete the medication dispensing system agreement, due 5 business days before orientation
Complete the RN skills self-assessment in DocuSign
Review and sign the RN and LVN orientation packet attestation
Review the Epidural Pain Policy
Review the Standard of Care: Skin Care packet and sign the attestation
Review the Sexual Violence and Sexual Harassment Policy
Sign the traveler acknowledgement
Complete orientation modules onsite, with the option of completing them at home after activating a network ID
