Required Qualifications

• Valid RN license in Iowa
• Med Surg experience: 1 year, with experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association
• NIHSS certification
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing

Health and Immunization Requirements

• COVID-19 vaccine: proof of the September 2025 booster, or a declination or exemption on agency letterhead. Declinations are not accepted in California or Illinois. End date of October 2026.
• MMR: proof of 2 vaccinations with dates, or a primary source IgG titer with lab printout
• Varicella: proof of 1 vaccination with date, or a primary source IgG titer with lab printout
• Hepatitis B: 3 doses, or a declination
• Influenza vaccine: proof, or a declination or exemption on agency letterhead. Declinations are not accepted in California or Illinois. Flu dates: 10/1/2025 - 4/30/2026.
• Tdap within the past 10 years. Declinations are accepted only where the site allows and the assignment does not involve pediatric interaction, using the agency form.
• TB screening: negative TSPOT or QuantiFERON within a year of start, or a 2-step PPD within the last year for both reads. A chest X-ray is accepted if results were positive within a year of start.
• Fit test: 3M 1860 or 1870
• Physical/health statement: exam within the past year documenting fitness to work
• Color vision test
• 9-panel drug screen (no THC) completed within 30 days of start, preferably within 14 days, and resulted with MRO signature before start

Background Checks and Compliance

• Criminal background check (federal, nationwide, and county, last 7 years) plus SSN verification, dated within 30 days of start. Maryland counties are included in the state search.
• National Sex Offender Public Website (NSOPW) search dated within 30 days of start. Michigan also requires a statewide search.
• Iowa criminal history, sex offender, dependent adult, and child abuse registry checks, completed within 30 days before start
• Child and adult dependent abuse training (Iowa only)
• Education verification by third-party screening (highest degree, or high school diploma or GED)
• EPLS exclusion check dated within 30 days of start
• OIG exclusion check dated within 30 days of start
• OFAC check completed within 30 days of start
• Conflict of interest disclosure form (page 1 signed)
• Confidentiality of Patient and Proprietary Information and Information System Security Agreement (page 5 signed)
• Code of conduct form (page 27 signed)
• Integrity and audit services form, signed within days before start
• System access form, completed one week before start
• Non-employee upload form (pages 1-3 and 28)
• Standard I-9, completed by the agency
• Badge photo
