Step Down RN - Cleveland, Ohio

Employment Type: Long-Term Assignment (LTA)
Contract Length: 13 weeks
Shift: 7:00 PM – 7:30 AM


Required Qualifications

  • License: Valid permanent RN license. Temporary licenses are not accepted. Must provide Primary Source Verification (PSV) and Nursys verification within 30 days prior to start. PSV must be updated every 2 years with license renewal.
  • Certifications:
    • BLS (American Heart Association only)
    • ACLS (American Heart Association only)
    • NIHSS certification
  • Health Requirements:
    • COVID-19 vaccine series proof preferred but not required (medical or religious exemption allowed)
    • Positive MMR titers within last 15 years or appropriate booster protocol
    • Positive Varicella titers within last 15 years or appropriate booster protocol
    • Tdap vaccination within last 10 years (no declinations accepted)
    • Hepatitis B: proof of full series, partial series, or signed declination (no childhood vaccines accepted)
    • 2-Step PPD or IGRA (Quantiferon/Tspot) with negative results within last 6 months; if positive, clear chest X-ray and current TB questionnaire required
    • Fit test for approved respirators completed within 1 year prior to start and updated annually
    • Physical exam within 1 year prior to start confirming ability to work without restrictions
  • Background and Verification:
    • Criminal background check within 6 months including Social Security trace, county searches (7 years), national criminal sweep, and sex offender registry
    • Ohio fingerprinting requirements based on residency duration (BCI&I and FBI checks) with attestation allowed if pending
    • Employment verification for last 5 years through third-party verification (no attestations)
    • Education verification through third-party; international education requires equivalency documentation
    • UHS I-9 attestation confirming eligibility to work in the U.S.
    • OIG exclusion check within 6 months prior to start
    • SAM/EPLS/GSA/OFAC screening within 6 months prior to start
  • Drug Screening:
    • 10-panel drug screen within 60 days prior to start (no instant tests; lab report required)
    • If break in assignment >60 days, new drug screen required
  • Competency and Skills:
    • UHS Skills Checklist completed within 1 year prior to start (clinical skills specific)
    • UHS Competency Exam with minimum 80% score within 1 year prior to start
    • UHS Core Competencies (HIPAA, Joint Commission-NPSG, OSHA topics) with minimum 80% score within 1 year prior to start
  • Identification:
    • Non-expired photo ID (Driver’s License or State ID)
  • Additional:
    • Badge photo (headshot, shoulders up, plain background, JPEG)
    • Submission packet including resume, 2 professional references (within last 2 years), job application, and employment history form

Additional Information

  • Prior travel experience is not explicitly required for this position.
  • Influenza vaccine is optional but recommended during flu season (October–March). If declined, masking and specific attestation forms are required.
