Required Qualifications
- Valid First Assist Technician license in Tennessee
- First Assist Technician (1 year of experience)
- First Assist Technician experience within the last year
- BLS issued by American Heart Association
- Driver's License
Onboarding / Compliance Requirements
- Ascension Core Competencies: HIPAA, Joint Commission, NPSG, OSHA Topics; completed within a year of start, updated every 3 years, minimum 80% passing score, only 2 attempts allowed.
- Ascension Surgical Technologist Contract Worker Onboarding Education Attestation: pre-hire modules must be completed prior to clearance.
- MMR: documented proof of two MMR vaccines or positive IGG titer; no declinations accepted.
- Fit Test: required for all travelers; preferred prior to start, can be completed onsite; updated annually.
- Hepatitis B: documentation of vaccination series completion or declination.
- Influenza Vaccine: exemptions/declinations accepted, but candidate must wear a mask at all times.
- Health Statement: statement of good health or completed physical exam prior to first assignment, updated annually.
- Driver's License/State ID: must not be expired and must show front/back.
- 2 Step PPD OR IGRA (Quantiferon/Tspot): TB testing and questionnaire requirements as stated.
- Varicella: documented proof of immunity/vaccination or provider verification; no declinations accepted.
- Tdap: valid for 10 years from administered date; no declinations or attestations accepted.
