Required Qualifications

• Valid Physical Therapist license in California
• BLS issued by American Heart Association or American Red Cross
• 1 year of employment verification related to the candidate’s profession
• Own transportation
• 2 professional references within 3 years of start
• Physical exam within 1 year of start
• 10-panel drug screen within 1 year of start
• Flu vaccine or declination required year-round
• Tdap within 10 years of start or annual declination
• Negative TB test or chest X-ray within 1 year of start
• Rubeola, mumps, rubella, and varicella immunity documentation as specified in the source
• Hepatitis B immunity documentation, vaccine series, or declination as specified in the source

Additional Details

• May visit up to 6 patients per day
• 27 productivity points standard
• Traveler must see 5–6 patients per day
• Weekends may be used to meet productivity
• Occasional maximum radius of 50+ miles
• Timely communication and documentation required
• Documentation tools provided and must be returned
• Any color scrubs
• Parking is free
