Role Summary

• Provide physical therapy assistant services in a rural, community-based home health program.
• Work collaboratively with physical therapists and the care team across a geographically diverse rural service area.

Required Qualifications

• Valid Physical Therapy Assistant license in Virginia
• BLS issued by American Heart Association or American Red Cross or Military or American Safety & Health Institute
• Valid and current driver’s license
• 2 professional references
• Application/resume required
• Skills checklist required
• E-Verify required
• Background check required
• MVR required
• Virginia State Police Check required and must be cleared prior to start
• TB screening documentation required
• Physical exam within the last 12 months
• Drug screen within the last 12 months
• COVID vaccine series preferred only; if not provided, a facility declination form is required
• Flu vaccination required during flu season, with approved medical or religious exemptions only
• Immunization documentation required for measles, mumps, rubella, hepatitis B, and varicella
• 7-year locations lived/worked background review required
• 3 years of driving history required
• Minimum age 21; 25 if transporting patients

• Mileage reimbursed at IRS rate
• Home health and hospice positions require a valid driver’s license
