Coverage Area

SF valley West Hills, North Hollywood, Glendale, Burbank, Northridge, Sherman Oaks
Glendale/Burbank/Tarzana/Pacoima/North/Hollywood/Northridge
May be asked to help in other territories within the facility’s service area as needed, about a 60-mile radius from the facility address

Required Qualifications

Valid Physical Therapist license in California
Experience in a Home Health setting
Experience with OASIS
Experience with Homecare Homebase

Work Expectations

Average 4-6 visits per day depending on type of visit for PT/OT/SLP
Productivity: 28-32 points per week for PT/OT/SLP
Overtime must be pre-approved and requires written approval from Administrator

Additional Notes

Client may consider candidates without HCHB experience at a reduced rate for the first 2 weeks if the manager is willing to train
2 weeks cancellation
