Guaranteed 36 hours weekly
May visit up to 6 patients per day

Area coverage includes Danville, Martinez, Moraga, Alamo, Clayton, Concord, Lafayette, Orinda, Pleasant Hill, Walnut Creek, Pittsburg, Discovery Bay, Byron, Antioch, Brentwood, Oakley, Bethel Island, and Bay Point
Locals accepted

On-call not required
Weekends may be used to meet productivity
Traveler must have their own transportation
Occasional maximum radius of 50+ miles

Valid Physical Therapy Assistant license in California
One completed travel assignment
BLS issued by American Heart Association or American Red Cross
Experience in a Home Health setting
Personal auto insurance
Auto registration
Physical exam within 1 year of start, with clearance to work with no restrictions/limitations or in good physical health
Negative TB or Chest X-Ray within 1 year of start
Tetanus/Diphtheria/Tdap within 10 years of start or annual declination
Rubeola (Measles): 2 vaccines or positive titers
Mumps: 2 vaccines or positive titers
Rubella: 2 vaccines or positive titers
Hepatitis B: titer within 5 years, vaccine series (3 doses), or declination
Varicella: 2 vaccines or positive titer
Drug screen within 1 year of start
Flu vaccine or declination
2 professional references within 3 years of start
