Physical exam within 1 year of start
Negative TB test or chest X-ray within 1 year of start, with annual update required
Drug screen within 1 year of start
COVID requirement applies; exemptions accepted
Flu vaccine or declination required during flu season
Measles, mumps, rubella, hepatitis B, and varicella documentation required
Tetanus/diphtheria/Tdap is not required

Orientation safety manual acknowledgement required annually
Competency exam required as applicable to profession/specialty
Medication test/pharmacology competency exam required as applicable to profession
Skills checklist required annually
Dress code policy, parking policy, confidentiality/security commitment, and emergency codes must be reviewed
Agency contract certification form must be completed before start
Fit testing will be completed onsite on the first day
