Required Qualifications

• Valid RN license in California
• Bachelor of Science in Nursing (BSN)
• Minimum 2 years of oncology experience
• Oncology experience within the last year
• Experience in an outpatient setting
• One completed travel assignment
• BLS certification issued by the American Heart Association or American Red Cross
• Chemotherapy certification (ONS/ONCC)

Pre-Start Requirements

• Background checks (within 60 days of start): Social Security trace, 7-year criminal history search, national sex offender search, FACIS Level 3 search, OFAC, EPLS/GSA/SAM, and HHS/OIG
• Education verification: Diploma or official transcripts
• Health documents:

  • Physical exam within the past year, updated annually
  • TB screening: QuantiFERON-Gold within one year of start (TSPOT and PPD not accepted)
  • Measles, mumps, rubella, hepatitis B, and varicella immunity (titer, proof of vaccines, or declination as applicable)
  • Tdap (declination or vaccine within 10 years)
  • COVID: one monovalent Pfizer or Moderna dose after 9/12/2023, or two Novavax doses (declination accepted)
  • Flu vaccine or declination, with documentation due by 11/01 during the 10/01 to 03/31 season
  • 9-panel drug screen within 12 months of start
  • Fit test with an approved N95 mask (onsite fit testing available for certain failed tests)
• Work history: Employment application covering the last 7 years, and work history verification for the last 5 years. Explain all gaps of 30 days or more.
• References: Two required, at supervisor level or above. One must be from the past year. Charge nurses are accepted.
• Skills checklist: Required
• Facility training: Completion certificates for required modules, including the California harassment prevention certificate (valid two years), code of conduct, radiation hazard awareness, safe patient handling, HIPAA, and annual education. Certificates must show name, date, and reference number.
• Signed forms (before start): Child, elder, and domestic abuse reporting acknowledgments, and confidentiality agreement
• Access forms: Badge/access forms are due within 72 hours of signing and must be submitted 3 Fridays before start. RNs must also complete a medication-system access form.
