• Location: Fresno, California
• Shift: 19:00 - 07:30
• Contract Length: 13 weeks
• Patient Population: Adults, Geriatric

Unit and Float Scope

• Levels of care: Tele (Telemetry), Med Surg, and ED Inpatient Hold
• Float requirement: Must float between all three facilities for any of these levels of care.
• Staffing counts listed: 1 Tele and 1 ED Inpatient Hold at two of the three facilities.
• Schedule: Given the day of, for the facility and level of care.
• Orientation: 4 shifts total.

Required Qualifications

• Current and valid Registered Nurse license to work within the state of California.
• Current Basic Life Support (BLS) for Healthcare Providers from the American Heart Association.
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN).
• Minimum of 2 years of float experience.
• Float experience within the last year.
• Valid driver's license.
• Resource experience.

Required Onboarding Documents

Before submission:

• Resume or profile
• 2 professional references, at least 1 supervisory
• Clinical skills checklist, completed within 1 year of start

Before start (on board):

• Proof of 2 MMR immunizations, positive IgG titer, or declination (negative titer requires booster or declination)
• Proof of 3 Hepatitis B vaccinations, positive antibody titer, or declination (negative titer requires booster or declination)
• Influenza vaccine, required October through March unless otherwise indicated by CDPH (medical or religious exemptions require approval)
• Proof of 2 Varicella vaccinations, positive IgG titer, or declination (negative titer requires booster or declination)
• Tdap vaccination within the past 10 years, or declination
• TB test within 30 days of start: negative TB skin test, QuantiFERON Gold, or T-spot, with results stated. Updated annually. Positive results require proof of positive history, chest x-ray within the last 5 years, and an annual TB questionnaire.
• Health statement signed and dated within 1 year of start, stating the ability to work without restrictions. Updated annually.
• Clear copy of valid driver's license or state ID (passport accepted)
• Criminal background check within 30 days prior to start, covering 7 years of county and national records for locations resided, employed, and attended school, plus SSN trace, employment verification (last 5 years), OFAC, and NSO. Accepted within 40 days if the start date is postponed. Updated every 3 years.
• Manual searches within 30 days of start, updated annually: OIG, EPLS, OFAC, National Sex Offender registry
• Medi-Cal Suspended and Ineligible Provider List search within 30 days prior to the first assignment, with a signed and dated screenshot documenting the search. Updated monthly.
• Drug screen within 30 days of start, 9-panel minus THC; rapid screen accepted. Accepted within 40 days if the start date is postponed. Updated annually. Retesting required if break in service exceeds 30 days.
• Signed and dated attestation of any Do Not Rehire/Send record from the prior 3 years, on the agency's letterhead
• Confirmation of assignment and job description
• Acknowledgement of receipt for the policy packet, with signature page
• Dispute resolution agreement, signed and dated
• Confidentiality of receipt form, signed and dated
• Release of information authorization for patient portal, initialed and signed
• Badge photo: neutral background, forward facing, no facial jewelry
