Required Qualifications

• Valid Ohio RN license
• Minimum of 12 months Med/Surg Telemetry experience, completed within the last year
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• BLS issued by American Heart Association or American Red Cross; RQI accepted
• ACLS (Advanced Cardiac Life Support) issued by American Heart Association or American Red Cross; RQI accepted
• NIH Stroke Certification (NIHSS) required (AHA, Apex, AACME, or Bluecloud only; HealthStream certificates not accepted)

Onboarding Requirements

• Background check: Required before start, updated for breaks in service of 2 or more months, and updated annually
• Fingerprinting: FBI and state (BCI) fingerprinting, completed within 48 hours of signed contract, with results within 12 months of start and updated annually. Candidates may start with results pending if fingerprinting is completed before start.
• Education verification: Required for all candidates
• Primary source license verification and Nursys verification: Completed within 30 days of start
• Driver's license or state ID: Government-issued photo ID required and must be carried on assignment
• Physical exam: Within 1 year of start, including a statement that the candidate can safely perform the essential job functions with or without reasonable accommodation
• TB screening: True 2-step PPD, QuantiFERON Gold, or T-SPOT within 12 months of start, updated annually. Candidates with a past positive need a chest X-ray within 5 years, plus an annual TB questionnaire.
• Measles, mumps, rubella: Two MMR vaccines or a positive titer. Negative or equivocal titers require one booster.
• Varicella: Two vaccines or a positive titer. Negative or equivocal titers require one booster.
• Hepatitis B: Titer, vaccine series, or declination
• Tdap: Required every 10 years, documented on an official immunization record
• Flu vaccination/documentation (2026-27): Due by November 15 for current candidates, or for new candidates onboarded between November 15 and March 31. No declinations accepted.
• Drug screen: Standard 10-panel within 12 months of start, updated annually. Instant drug screens not accepted.
• Fit test: 3M N95 1860 small/regular or 1870+, within 12 months of start and updated annually. Cannot be completed onsite.
• Eye exam: Snellen eye exam valid within 1 year
• References: 2 required, from employment within the past year, from a manager, ANM, supervisor, charge nurse, or director. Co-worker references not accepted.
• Employment application: Must show 3 years of prior work history, including at least 2 years of clinical practice in an acute care setting in a similar area to the assignment
• Skills checklist: Required annually
• Online training modules: Must be completed before start, on a computer using Google Chrome (not a phone)
