Required Qualifications

• Current, valid Registered Nurse (RN) license in California
• Associate of Science in Nursing (ASN) or Associate Degree in Nursing (ADN)
• 2 years of Med Surg / Telemetry experience, with experience within the last year
• Current Basic Life Support (BLS) for Healthcare Providers issued by the American Heart Association (AHA)
• Driver's license
• Resource experience

Onboarding Requirements

Required for submission:

• Resume or profile
• 2 professional references, at least 1 supervisory
• Clinical skills checklist, completed within 1 year of start

Required before start:

• MMR: proof of 2 immunizations, positive IgG titer, or declination
• Hepatitis B: proof of 3 vaccinations, positive antibody titer, or declination (booster required if titer is negative)
• Varicella: proof of 2 vaccinations, positive IgG titer, or declination (booster required if titer is negative)
• Tdap: vaccination within the past 10 years, or declination
• Influenza vaccine, required October through March unless otherwise indicated by CDPH; medical or religious exemptions accepted with documentation
• TB test negative within 30 days of start, updated annually (if positive: proof of positive history, chest X-ray within the last 5 years, and annual TB questionnaire)
• Signed and dated health statement within 1 year of start, confirming the ability to work without restrictions, updated annually
• Driver's license or state ID (passport accepted)
• Criminal background check within 30 days prior to start, including 7-year county and national searches, SSN trace, 5-year employment verification, OFAC, and NSO
• Manual searches within 30 days of start, updated annually: OIG, EPLS, OFAC, National Sex Offender registry
• California Medi-Cal Suspended and Ineligible Provider List check, completed within 30 days before the first assignment and updated monthly thereafter
• Drug screen within 30 days of start (9-panel minus THC; rapid screen accepted; retesting required if break in service exceeds 30 days)
• Signed and dated assignment confirmation/job description
• Signed policy packet acknowledgment
• Signed dispute resolution agreement
• Badge photo (neutral background, forward-facing, no facial jewelry)
• Signed release of information form for the patient portal
• Attestation of no Do Not Rehire/Send record from the prior 3 years, on agency letterhead
• Signed confidentiality form

Optional:

• COVID-19 vaccine (proof of vaccine and 1 booster, or exemption)
• Fit test, completed onsite on start date
• Pre-start testing, to be completed by the Friday before start
