Location: Clarksville, Tennessee Employment Type: Temporary Contract Length: 13 weeks Hours: 8 hours per day, 40 hours per week Schedule: 8:00 AM to 4:30 PM, with on-call requirements Patient Care Setting: Hospice
Role Overview
- Sees approximately 7 to 8 patients daily, though this may fluctuate.
- Covers a 6-county service area. Staff are generally assigned a general area when possible, but areas may change based on census and daily needs.
- Responsibilities include assessing and managing patient care, medication reconciliation and refills, weekly IDG notes, attending weekly IDG meetings, communicating with MDs about patient condition and needs, and completing charting.
Required Qualifications
- Valid RN license in Tennessee
- Minimum of 1 year of hospice experience, with hospice experience within the last year
- Associate of Science in Nursing or Associate Degree in Nursing
- Experience with Homecare Homebase

Dress Code
- Gray or black scrubs preferred; avoid anything too bright.
