Assignment Overview

Unit Details

• Centralized pharmacy model
• Hospital has a pneumatic tube system
• EMR software familiarity is required (software familiarity, not a credential)

Traveler Policies

• Accepts travelers with one year of experience
• Accepts first-time travelers if they have hospital experience from elsewhere

Required Qualifications

• Valid Hospital Pharmacist license in Nevada
• At least one year of hospital pharmacist experience, completed within the last year
• Experience with Epic

Health and Immunization Requirements

• Physical exam: Required within 12 months of start (Nevada state mandate; cannot be waived)
• TB screening: QuantiFERON-Gold, T-SPOT, or 2 PPDs within 1 year of start. Facility has been low-risk since 2020, and annual TB testing is not required.
• TB questionnaire: Required annually (Nevada state mandate)
• Immunity documentation (Rubeola, Mumps, Rubella, Varicella): Positive titer or 2 vaccines. Low titers have additional booster requirements.
• Hepatitis B: Positive titer or signed declination. Low titers require a 2- or 3-dose series or declination.
• Seasonal flu vaccination: Documentation required by November 15. Declinations are not accepted. Religious or medical exemptions are reviewed case by case and must be approved by the client.
• Drug screen: Negative 10- or 11-panel drug screen (must include marijuana) within 30 days of start. Point-of-care tests are acceptable. No annual drug screen required.
• Fit test: N95 fit test (3M 1860 small/regular, 1870+, 9205, or 1804 small/regular). Candidate schedules their own appointment.

Background and Verification

• Standard background check: county searches for the last 7 years of locations lived, worked, or in school; nationwide sex offender search. An international search is required if the candidate has spent one month or more in a foreign country.
• EPLS/GSA/SAM and HHS/OIG checks, completed annually
• Education verification: transcript or diploma is preferred for this role (not required)
• Driver's license or state ID (government-issued photo ID)
• Pending background check attestation required if fingerprints will be pending at start
