RN Case Manager-Hospice

Location: Sunnyvale, California Employment Type: Contract Specialty: Nursing Contract Length: 13 weeks Hours per Week: 40

Position Summary

The Hospice Nurse admits patients to service and assesses the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. The nurse develops an individualized plan of care, documents observations and interventions related to the patient's environment, coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment and performance improvement program, and conducts follow-up visits as needed.

Required Qualifications

• Valid RN license in California
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing
• 1 year of hospice nursing experience, including hospice experience within the last year

Schedule

• Days: 8:30am-5pm (schedule can be flexible based on patient availability)
• Weekends: One weekend per month

Territory

Oakland, Alameda, San Leandro, Ashland, Castro Valley, San Lorenzo, Hayward, Union City, Fremont, Newark, Milpitas, and neighboring cities as needed.
