Assess patient conditions, implement care plans, and provide ongoing education and support to patients and their families.
Perform admission processes and case management responsibilities under home care policies and standards of practice.
Complete thorough, timely electronic documentation and interdisciplinary communication.
Travel to cases as assigned.
Weekend rotations occur once every 6 weeks.

Valid Physical Therapist license in Massachusetts
Physical Therapist with 1 year of experience, including experience within the last year
Experience in a Home Care or Home Health setting
1 year of Case Management experience
