ICU Registered Nurse (Travel/Contract)

Location: Saint Marys, Ohio Unit: M/S Unit, 3 ICU beds Start date: ASAP Contract length: 13 weeks Shift: Night, 3x12-hour shifts, 19:00 to 07:00 (36 hours per week) Weekends: Every other weekend On call: Not required

Assignment Details

• Floating: May float to the ICU/SD (Stepdown) unit as needed. Stepdown experience is required for floating.
• Block scheduling: Willing to discuss during interview.
• Traveler eligibility: First-time travelers accepted. Local candidates not allowed. 50-mile radius restriction.

Required Qualifications

• Valid RN license in Ohio
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• ICU (Intensive Care Unit) experience: 1 year, with experience within the last year
• Stepdown experience
• ACLS (Advanced Cardiac Life Support) issued by American Heart Association, American Safety and Health Institute, American Red Cross, or RQI
• BLS (Basic Life Support) issued by American Heart Association, American Safety and Health Institute, American Red Cross, or RQI
• PALS (Pediatric Advanced Life Support) issued by American Heart Association, American Safety and Health Institute, American Red Cross, or RQI

Pre-Start Requirements

Background and eligibility

• Criminal background check within 6 months of start, including a FCRA-compliant 7-year search (SSN trace, counties, statewide, nationwide criminal, national sex offender, OFAC, FACIS Level 3)
• EPLS/GSA/SAM and HHS/OIG checks
• BCI and FBI fingerprinting within 6 months of start, updated annually. Candidates may start with results pending. Fingerprints must be redone if the traveler has lived or worked outside Ohio since their last fingerprinting.
• Fingerprint clearance attestation within 6 months of start
• Results are mailed, so complete fingerprinting as early as possible.

Health and immunizations

• Pre-employment physical within 30 days of start (not annual)
• Negative 10-panel drug screen within 30 days of start, including THC (not annual)
• Negative TB test within 30 days of start: 2-step PPD, T-Spot, or QGold (not annual)
• TB questionnaire, updated annually. Past positive TB candidates must also provide an annual symptom assessment signed by a physician, NP, or PA.
• Tdap within 10 years
• Hepatitis B: titer, proof of vaccine series, or facility-specific declination
• Measles, mumps, rubella: 2 MMR vaccines or positive titer. Negative or equivocal titers require 2 boosters, and the second must be completed 28 days after the first. A pending second booster is acceptable at start.
• Varicella: 2 vaccines or positive titer, with the same booster rule as above
• Flu vaccination required between 10/1 and 3/31. Declinations are not accepted. Medical and religious exemptions require facility-specific forms approved in advance. Flu vaccines may be given on site if not completed before start.
• Facility-specific medical or religious COVID exemption forms, if applicable
• Facility-specific declination forms for Hep B, Tdap, or COVID vaccines, if applicable

Facility documents

• Photo for badge: headshot on a white background, with no hats, caps, or head coverings
• Confidentiality statement, to be reviewed and signed
• Facility-specific verification form, to be completed before the first day
• Fit test, completed onsite during orientation
