Required Qualifications

Licensure and certifications

• State license with primary source verification
• Basic Life Support (BLS), required for all candidates, from the American Heart Association or American Red Cross (AHA Provider, Instructor, and RQI cards are accepted)
• Advanced Cardiac Life Support (ACLS), American Heart Association or American Red Cross, required depending on the unit description
• Pediatric Advanced Life Support (PALS), American Heart Association or American Red Cross, required depending on the unit description
• Neonatal Resuscitation Procedure (NRP), required as applicable per the unit description
• NIH Stroke Certification, required as applicable per the unit description
• Crisis Prevention Institute (CPI) training, required as applicable per the unit description
• Trauma Nursing Core Course (TNCC), required for ER RNs

Identification and background

Competencies and documentation

• Competency exam, required as applicable by profession and specialty. Minimum score of 80%
• Medication Test/Pharmacology Competency Exam, required as applicable by profession. Minimum score of 80%
• Orientation Safety Manual acknowledgment, annually
• Skills checklist, annually
• 2 professional references, within 3 years of start

Health Requirements

Most items below are time-sensitive and must be completed within 72 hours of signing the contract. Candidates who decline a vaccine must complete the facility's declination form.

• Physical exam: Required within 1 year of start. Not required annually.
• Tetanus/Diphtheria/Tdap: Tdap within 10 years of start, or a declination.
• TB screening: Negative PPD, TSPOT, or QuantiFERON-Gold within 1 year of start, repeated annually. If the result is positive, a negative chest X-ray within 5 years of start (dated after the positive result) and an annual TB questionnaire are required.
• Measles, mumps, and rubella: Positive titer, or 2 MMR doses at least 28 days apart. For negative or equivocal titers, options include a declination, a booster, or 2 vaccines. Candidates born before 1957 may be exempt under the 1957 rule, which the agency confirms with the client.
• Hepatitis B: Positive titer or completed vaccine series. A non-immune titer requires a booster or a declination.
• Varicella: Positive titer, 2 vaccines at least 4 weeks apart, or a history of chickenpox. For negative or equivocal titers, options include a declination, a booster, or 2 vaccines.
• Drug testing: Negative 10-panel drug screen within 1 year of start. Not required annually.
• Flu: Vaccine or declination required during flu season (10/01 to 03/31). Documentation is due by 11/30. Free flu clinics are available.
• COVID-19: Vaccine or declination required.
