Schedule

• Shift: Day, 5 x 8 hours (8:00 to 5:00)
• Hours: 8 hours per day, 40 hours per week

Required Qualifications

• Valid Physical Therapy Assistant license in Ohio
• BLS (Basic Life Support) certification required

Health and Screening Requirements

• Background check: Required before start, with timing by state: within 30 days of hire (IL, IN, KY), at time of offer (MI, OH), at time of hire (VA), or prior to employment (MO)
• Fingerprinting: Required for all candidates, completed by the state timing above, or a fingerprint receipt provided before start with results submitted once received
• TB screening: 2-step PPD, Quantiferon Gold, or T-SPOT within 1 year prior to start; if prior positive, negative chest x-ray within the last 5 years
• Measles, mumps, rubella, and varicella: 2 vaccines or positive titer required (direct workers). Negative or equivocal titers may be resolved by declination, 2 vaccines, or 1 booster. Candidates born before 1957 are exempt.
• Hepatitis B: Facility-specific Hep B form required. If accepting, proof of vaccination is required. If declining, no further documentation is needed.
• Physical exam: Required for candidates working in Kentucky, on facility form. Direct workers must provide a physical statement within 12 months of start.
• Drug testing: 5-panel urine drug screen required before start for home health and hospice candidates working in Indiana, including home health aides. 9-panel drug screen required for direct workers.
• COVID-19: Vaccination plus 1 booster required for candidates in Virginia. Medical or religious exemptions accepted on facility form with approval.
• Flu: Vaccine or exemption required during flu season (November to April) for home health and hospice candidates at certain locations. Exemptions must be on facility form and sent for approval.
• Certification of good health: Required for candidates working in Kentucky
• Badge photo: Required, except at certain locations

State-Specific Requirements

• Michigan: LARA fingerprint results, completed at time of offer
• Missouri: Highway Patrol check results, completed prior to employment
• Indiana: Fingerprint results, completed within 30 days of hire
• Virginia: VSP national criminal background check fingerprint results, completed at time of hire
• Ohio: BCI/FBI fingerprint result attestation, completed at time of offer
