Telemetry Registered Nurse (Per Diem)

Location: Placentia, California Employment type: Per diem Contract length: 13 weeks Shift: Night, 1 x 12 hours (7:00 PM to 7:30 AM) Hours: 12 hours per shift; 12 hours per week listed

Role Overview

• Unit: Telemetry
• Float: May float to MS (Med Surg)
• Patient ratio: 1:5
• Patient population: Adults and geriatrics
• Unit metrics: Average daily census 23; 5 visits; 16 rooms; 30 beds
• Systems: Epic (software familiarity); utilizes Omnicell

Required Qualifications

• Experience: 2 years of experience
• Certifications:

  • ACLS (AHA or RQI only; instructor cards not accepted)
  • BLS (AHA or RQI only; instructor cards not accepted)
  • NIHSS
  • Note: The certification section states NIH is not required for Tele RNs, which conflicts with the requirements list. Confirm with the facility.
• Licensure: State license verification (primary source)
• Continuing education: Copies of CE completed within the last 24 months, each showing at least 1 contact hour. BLS cannot be used for CE credit.
• References: 2 references from the last year in the current specialty, from a leadership role (Department Manager, Supervisor, Charge Nurse, Nurse Manager, or Assistant Nurse Manager) who has supervised the candidate in a work assignment.
• Competency exams:

  • Medication/calculation exam
  • Unit-specific competency exam
  • Basic Dysrhythmia (EKG) exam
  • Arrhythmia Recognition Assessment with an 80% passing score (10 strips; must correctly identify ventricular fibrillation, ventricular tachycardia, third-degree heart block, and asystole with correct interventions)
  • Infection Control/Bloodborne Pathogens exam with an 80% passing score (the facility does not accept CORE exams in its place)
• Background: Criminal background check within 1 year of start, updated annually (7-year search); government-issued photo ID
• Health and immunizations:

  • Physical exam within 1 year of start, updated annually
  • N95 fit test within 1 year, renewed annually (PAPR accepted only with documented failed N95 fit test)
  • Negative TB test (PPD, TSpot, or QuantiFERON) within 1 year, or a negative chest X-ray within 4 years plus an annual TB questionnaire if positive
  • Rubeola, mumps, rubella, and varicella: positive titer or 2 vaccines after age 1
  • Hepatitis B series or declination (titer not accepted)
  • 9-panel drug screen within 1 year (instant drug screen acceptable; not updated annually)
  • Tetanus/diphtheria/Tdap
  • Flu vaccine by 12/5, or declination on file; declining candidates must mask at all times during flu season
• Availability and residence: Must provide at least 1 day of availability per week and live within a 50-mile radius
• Prior separation: 1-year separation from the health system

Preferred Qualifications

• Certifications: CPI (Blue Card only)
• Skills: ER holding, interpretation of arterial blood gases (ABGs), tracheostomy care and management

• Cardiac monitoring and emergency response; cardiovascular; IV insulin protocols; gastrointestinal; feeding tube care and management; genitourinary/renal; IV therapy/phlebotomy; heparin protocols; managing cardiac drips (no titration); managing vasoactive drips (no titration); PCA; CVA; post-TPA care; seizure disorders; substance withdrawal; total joint replacement; traction; surgical drains; respiratory

Orientation and Onboarding

• Orientation: 1 day shift scheduled based on availability (Monday to Friday), 12 hours on unit
• Pre-start modules: 12 to 14 hours
• Epic: Remote Epic test-out; if not passed, Epic modules are required
• Documents to review or sign: Confidentiality agreement (within 72 hours of signing), job description, orientation packet, ICU/traveler tip sheet (if applicable), skin care standard of care, epidural pain policy, and workplace violence acknowledgement (per diem)
