Surgical Technician

Location: Mason City, Iowa Employment Type: LTA Contract Length: 13 weeks Shift: 06:30 - 15:00

Schedule

• On call every 4th weekend
• 1-2 late shifts per week (1500-1900)

Required Qualifications

• Certified Surgical Technologist (CST) or NBSTSA certification
• At least 1 year of experience as a Certified Surgical Technologist, including experience within the last year
• Basic Life Support (BLS) certification issued by the American Heart Association

Onboarding and Submission Requirements

Submitted with application:

• Resume, skills checklist (within the past year), and 2 professional references

Health and immunizations:

• Physical/health statement from an exam within the past year
• TB test: 2-step PPD or IGRA (QuantiFERON/T-SPOT), negative within the past year. A chest X-ray is accepted if the result was positive within the past year.
• Drug screen (panel) completed and resulted before start, within 30 days of start, with MRO signature. THC is not included in the panel.
• Color vision test
• Fit test (1860 S and R and 1870s)
• Vaccines or proof of immunity: MMR (2 doses or IgG titer), Varicella (1 dose or IgG titer), Hepatitis B (3 doses or declination), Tdap within 10 years (declination accepted only where the site allows it), influenza for the 2024-2025 and 2025-2026 seasons, and COVID-19 (Sept 2025 booster, declination, or exemption on agency letterhead)

Background and screening:

• Federal, nationwide, and county criminal background check covering the last 7 years, plus SSN verification, dated within 30 days of start
• Iowa criminal history, sex offender, and dependent adult and child abuse registry check, completed within 30 days before start
• Iowa child and adult dependent abuse training
• Exclusion checks (EPLS, GSA/OIG, NSO, and OFAC), each dated within 30 days of start
• Education verification (highest degree, or high school diploma/GED)

Forms and administrative:

• Standard I-9 completed by the agency
• Code of conduct form (page 27), confidentiality agreement (page 5), conflict of interest disclosure (page 1), and non-employee upload form (pages 1-3 and 28)
• Integrity and audit services form, signed within days before start
• System access form, completed one week before start
• Badge photo
