Location: Chippewa Falls, Wisconsin Specialty: Therapy/Rehabilitation Employment Type: Travel/Contract Contract Length: 13 weeks Shift: Day (5x8 hours), 8:00 to 17:00 Hours: 40 guaranteed hours per week (8 hours per day)

Role Details

• Home Health OT specialty
• Case types: Geriatric
• Patients per day: 5-7 (to be confirmed during interview)
• First-time travelers: Accepted
• Locals: Accepted
• Territory: Up to a 60-mile radius from the hub/office
• On-call: Required every 3rd to 4th weekend
• Equipment: iPad provided
• Cell phone: No

Required Qualifications

• Valid Occupational Therapist license in Wisconsin
• Minimum 1 year of Occupational Therapist experience, completed within the last year
• One completed travel assignment
• Basic Life Support (BLS) certification (AHA, ARC, or Military accepted)
• Valid driver's license
• Experience with OASIS
• Experience with Homecare Homebase

Background and Compliance

• Background check within 90 days of start, updated annually, including SSN trace, county searches for the last 7 years, FACIS L1M, nationwide sex offender search, motor vehicle records, education verification, and work history verification
• EPLS/GSA/SAM and HHS/OIG checks within 90 days of start
• Education verification of highest degree related to profession
• Competency exam as applicable to profession and specialty
• Orientation safety manual acknowledgement, updated annually
• Acceptable use policy, completed before start (time sensitive)
• Work history: 7 years verified, with no gaps in the application
• Two professional references (travel), completed within 2 years of start
• Skills checklist, required annually

Health Requirements

• Physical exam statement within the last 12 months
• Fit test (N95 accepted, no model preference)
• Negative TB test within 1 year of start, with annual TB questionnaire
• Measles, mumps, rubella, hepatitis B, and varicella immunity documentation
• 9-panel drug screen within the last 12 months
• Flu vaccination or declination for the flu season
• COVID vaccination or declination

Work Expectations

• Dress code: Scrubs in any color; no jeans
