Stepdown / Immediate Care Unit RN

Schedule and Assignment

• Shift: 12-hour shifts (48 hours/week). The shift field lists a day shift of 7:00 to 19:00 (4x12). The additional info lists nights of 7 p.m. to 7 a.m. (3x12). Both are stated as weekdays with some weekends.
• Ratio: Typically 1:4, flexing to 1:5 in emergencies.
• Blocked schedules: Considered for weekend blocks only (Fri/Sat/Sun or Sat/Sun/Mon).
• Holiday requests: Limited to no more than 2 of the 4 major holidays.
• Unit patients: Many patients would be Progressive Care or Tele patients at a larger facility. Acuity will vary.
• Scrubs: Not provided. Burgundy and/or white.
• Travel experience: Not required.

Required Qualifications

• Valid RN license in West Virginia
• Stepdown experience (2 years)
• Stepdown experience within the last year
• ACLS (Advanced Cardiac Life Support) issued by the American Heart Association
• BLS (Basic Life Support) issued by the American Heart Association
• NIHSS (NIH Stroke Scale) certification
• Associate of Science in Nursing (ASN) / Associate Degree in Nursing (ADN)
• IV insertion skills
• Med Surg / Telemetry experience

Background and Credentialing

• 7-year criminal background check, completed within a year of start and updated annually. Must cover all locations lived or worked in the past 7 years.
• EPLS/GSA/SAM and HHS/OIG checks, completed within 30 days of start and updated at each renewal or extension.
• Education verification required.
• E-Verify proof or attestation required.
• Government-issued photo ID required, and it must be carried during all hours of service.

Health and Immunizations

• Initial physical exam, updated annually, attesting to ability to meet the job's physical demands.
• Tdap within the past 10 years.
• TB: negative 2-step PPD, QuantiFERON, or T-SPOT within 1 year of start. Chest X-ray within 2 years and a TB questionnaire are required for candidates with a history of positive TB.
• Measles, mumps, and rubella: titer or 2 vaccines at least 28 days apart.
• Hepatitis B: titer, 3-series vaccine, or declination.
• Varicella: history of chickenpox, titer, or 2 vaccines at least 28 days apart.
• 10-panel drug test within 30 days of start and updated annually. Instant drug screens are not permitted.
• Flu vaccine or agency medical/religious exemption required during flu season (10/12 to 04/01). Declinations are not permitted. Vaccines are available on site starting 10/12. Deadline is 11/11.
• Color blind exam required.
• COVID vaccine or declination required.

Submission Requirements

Submit one PDF containing:

• Candidate profile (work history)
• Specialty-specific skills checklist (updated annually)
• Minimum 2 manager/charge references from the past 2 years
• RN license verification (NURSYS)
• Copies of all required certifications

Two clinical references are also required, and a hospital evaluation is accepted in place of one.

Onboarding

• Badge photo, required before clearance.
• Background drug screen consent form, completed before start.
• Confidentiality (non-disclosure) form, completed before start.
• Facility checklist, completed before start.
• Parking pass form, completed before start.
• Care learning modules, due by Friday at 4 PM EST before start. Required at this facility.
• Employee health online surveys, with a screenshot of completion, due by the facility's hard document deadline.
• Orientation safety manual acknowledgment and core competency testing (Parts I to III). Subcontractors may submit a signed HIPAA form (preferred) or complete core testing.
• Preferred, not required: workers' compensation documentation.
