License/Certificate Verification requires current license and primary source verification within 30 days prior to start
Ascension Specialty Exam must be completed prior to first assignment and updated every 3 years
Ascension Core Competencies must be completed within a year of start and updated every 3 years
Fit Test is required for all travelers and must be updated annually
Influenza Vaccine exemptions/declinations are accepted, but a mask must be worn at all times
Varicella and MMR declinations are not accepted
Tdap is valid for 10 years from the administered date
Ascension Skills Checklist must be completed within one year of start and updated annually
Ascension 11 Panel Drug Screen must be completed within 30 days prior to the start of the first assignment and updated annually
